«٣١ آب - أغسطس ٢٠١٠» كان أحد الغواصين يقوم بأعمال صيانة دورية لبركة وقود مستهلك في مفاعل ليبشتات في سويسرا. رصد قطعة أنبوب غير معروفة ماهيتها في قاع البركة. أبلغ رئیسه عن ذلك بواسطة الراديو لمعرفة ما ينبغي فعله، فطلب منه وضعه في سلة أدواته، وقد فعل ذلك. ونتيجة لضجيج الفقاعات في البركة لم يسمع منبهه الإشعاعي. عندما رُفعت سلة المعدات من الماء، انطلقت منبهات الإشعاع في الغرفة. وتم إعادة إلقاء السلة في الماء وغادر الغواص…
