المشكله بدأت لما المريض جاله وجع في صدره وأخد داينترا (Isosorbide dinitrate).. الـ النيترات دي شغلها الأساسي إنها بتتحول في الجسم لـ Nitric Oxide وده بدوره بيروح يحفز إنزيم تاني اسمه Guanylate Cyclase عشان يصنع كميات مهولة من الـ cGMP وبتزود إنتاج الـ cGMP بمعدلات بتوصل لـ 10 أضعاف المعدل الطبيعي وفي نفس الوقت "المقص" (PDE5) مقتول بفعل الفياجرا، فبيحصل تراكم للـcGMP جوه الخلية، وده بيعمل Extreme Smooth Muscle Relaxat…

مرة في إمتحان الشفوي دخلت لدكتور صعب جداً، سألني سؤال صعب فمعرفتش أجاوبه، زَ.....عق وشرحهولي بالتفصيل، وبعدين جاتله مكالمة طويلة ونساني، بعد ما خلصها رجع سألني نفس السؤال، جاوبت نفس الإجابة اللي قالهالي وفاكره بيراجع معايا.. إنبهر بيا وقالي براڤوو عليك واداني الدرجة كاملة وروحت.
