Update The issue has been caused by some NVMe disks that were being rejected by Ceph. After checking the NVMe status, all disks are confirmed to be fully healthy. This appears to be a software-related issue. We have temporarily bypassed and re-enabled disk synchronization. Services should return to full operation within the next few minutes.
